Acceptance
By using forms.wtf you agree to these terms. If you do not agree, do not use the service.
Service Description
forms.wtf is a form building platform with wallet verification, token gating, and team collaboration.
Accounts
- You are responsible for your wallet security.
- One account per person/entity.
- You must be 18+ or have legal capacity.
Acceptable Use
- Don't use forms for illegal purposes.
- Don't collect data in violation of privacy laws.
- Don't spam, phish, or deceive respondents.
- Don't attempt to bypass token gating or rate limits.
- Don't reverse engineer or scrape the service.
Form Creator Responsibilities
- You are responsible for your form content and data collection practices.
- You must comply with applicable privacy laws (GDPR, CCPA, etc.).
- You must inform respondents about data collection.
- Token gating rules must be accurate and fair.
Respondent Data
- Wallet addresses submitted are visible to form creators.
- Responses are stored in our database and optionally synced to Google Sheets/webhooks.
- One submission per wallet per form (can be updated).
Payments & Subscriptions
- Free tier available with usage limits.
- Paid plans billed monthly via Stripe or cryptocurrency.
- Crypto payments are non-refundable once confirmed on-chain.
- Stripe refunds handled per Stripe's policies.
- We reserve the right to change pricing with 30 days notice.
Intellectual Property
- You own your form content and response data.
- We own the platform, brand, and technology.
- You grant us a license to host and display your forms.
API & Webhooks
- API access subject to rate limits.
- Webhook URLs must be HTTPS in production.
- We are not responsible for webhook endpoint failures.
Availability
- Service provided “as is”.
- No guaranteed uptime SLA for free plans.
- Business plans include reasonable availability commitment.
Termination
- You can delete your account at any time.
- We can suspend accounts violating these terms.
- Data export available before deletion.
Limitation of Liability
- We are not liable for on-chain transaction failures.
- We are not liable for smart contract vulnerabilities in gated contracts.
- Maximum liability limited to fees paid in the last 12 months.
Changes
We may update these terms. Continued use of the service after changes constitutes acceptance of the updated terms.
